Job Summary

The Patient Diabetic Educator will work with patients to help them manage their disease by providing education, support and counseling on diet, exercise, and medication use

Job Responsibilities 1

Communicates with patients to explain their disease and answer their questions.

Explains treatment plans to their medical teams and patients.

Performs clinical assessments, such as measuring height and weight, taking blood pressure, checking feet for signs of injury or infection, and administering tests for glycated hemoglobin levels.

Uses empathy to help their patients understand the challenges they may face as a result of their condition.

Provides individualized instruction to patients on how to monitor their blood sugar levels and adjust their diets accordingly.

Educates patients on how to prevent and control the progression of the disease and how to adjust their life for that purpose.

Educates patients on how to inject insulin or other medications using proper techniques to avoid infections or other complications.

Job Responsibilities 2

Educates patients on basic principles of nutrition, meal planning, food selection, vitamins, and minerals, and on how to read food labels.

Educate patients on necessary follow ups with physicians.

Uses the teach-back method by asking follow-up questions to patients once s/he explains a process or concept to them to make sure they have fully grasped the information.

Reviews patient records to determine whether the patient is meeting treatment goals.

Provides patients with information about community resources such as support groups or social service agencies that can help them manage their conditions.

Coordinates and facilitates support groups for patients who have been diagnosed with diabetes or other endocrine disorders.

Maintains an inventory of supplies such as glucose monitors, insulin pumps, lancets, syringes, test strips and other items used in diabetes care.

Stays abreast of latest treatment methods and advancement in diabetes research.

Minimum of 4 years of experience as a Diabetic Educator (required)
